What is a Geriatric Care Manager?
Geriatric Care Managers are nursing or social work specialists who work with families and seniors to assist them in learning how to care for and plan for the later years in life. We can assess seniors’ needs and coordinate care to help them maintain their well being, independence and dignity. We can also help lead family discussions about sensitive subjects, offering our expertise so families can make informed decisions.

When should we start planning for the future? My parents seem to be in good health now.
Many people think if everything is okay, why get involved with their parents’ affairs? But looking for help with aging parents when everything is okay is actually the best time to have this conversation—everyone is calm and wants to talk. Don’t wait until there is a crisis. Having a plan in place relieves everyone’s stress, lets everyone involved be in on the decision-making process and allows seniors to have their preferences known.
What sort of care plans are available? How can I know all of my or my loved one’s needs will be met?
When looking for services to assist elderly parents, Senior Care Concepts can coordinate as much or as little care as you need. From helping with monthly bill-paying and setting up doctor’s appointments, to coordinating around-the-clock in-home care and finding contractors for home safety and mobility improvements, we’re here to help. No two clients are alike, so we take the time to personally get to know you and listen to your unique needs. We can coordinate both short- and long-term plans, or even help you through a crisis situation.
I’m not sure how to bring up an assisted living facility to my aging parents. How can I approach this sensitive subject?
