Barbara
Barbara was living alone when she fell and broke her hip. While she was in the hospital, the staff realized Barbara’s son lived out of state and that she didn’t have nearby family to assist her. The staff recommended she call Jenny to help with her care, who was able to set up a Philips Lifeline in Barbara’s home in the event of a future crisis. She also helps Barbara set up doctor’s appointments and arranges weekly visits from a Senior Care Concepts nurse who assists with Barbara’s medication management and rehabilitation. Every week, Jenny calls or stops by Barbara’s house to make sure she has plenty of groceries, feels and looks good, and has everything she needs, reporting her condition back to Barbara’s son.
Bob and Sally
Bob and Sally live at home, but this past Thanksgiving their children noticed Sally’s dementia getting worse. Her deteriorating health was affecting her day-to-day functions, also making it increasingly difficult for Bob to care for her. Feeling clueless about assisted living communities and in-home health care, the family called Jenny to discuss their options. Jenny arranged for Sally to enroll in an adult day center, where she now goes three days a week. This change has allowed Sally a safe and social place to be, while relieving Bob’s stress and allowing him to take care of himself. Bob and Sally additionally receive weekly visits for added assistance around the house. Jenny was also able to talk the family through long-term planning, putting procedures into place if things worsened in the future. The family now has peace of mind knowing which assisted living community their parents will go when they can no longer live safely at home.
Henry and Joan
When Joan was diagnosed with an aggressive form of Parkinson’s Disease, she and her husband Henry knew they needed help determining the best course of action. Jenny was able to assist them in forming both short- and long-term care plans. Henry and Joan wanted to stay in their home as long as possible, so Jenny walked through their house, making suggestions in every room to create a safer environment and easier mobility for Joan. Because Henry worked during the day, Jenny installed a Philips Lifeline in their home and arranged for a companion to help Joan with daily tasks. As an active member in her town’s choral group, Jenny also coordinated transportation to and from practices, and set up appointments with a speech therapist to help Joan maintain control over her voice. Once Joan’s health got worse, Jenny was there to coordinate in-home medical care, find hospice care and guide them through the process of making funeral arrangements. Checking off every goal on her long-term checklist along the way, Joan and Henry were able to take comfort that they were in control of her care every step of the way. At Joan’s funeral Henry specially thanked Jenny for her passion and for being there for them to make the most of their final years together.
