My experience in senior care planning and management has given me extensive knowledge of the many assisted living and rehabilitation facilities in the state, access to dozens of physicians and specialists, and connections with a variety of at home service providers.
For over 18 years, I have been providing in-depth care assessments and long-term health care planning for seniors, also acting as a liaison between health care professionals, seniors and their families. I have worked at local assisted living communities in social work and community relations, learning how best to accommodate seniors with varying levels of need, as well as their families.
In addition I have an extensive background in end-of-life planning. This allows clients to have a sense of control and peace of mind knowing that their wishes and preferences will be respected. Support continues to emotionally assist the family during the grieving process.

As a certified care manager, my goal is to assist families and help plan for life's changes. We understand the hardships, so our team works hard to anticipate the needs of our clients and their loved ones. Most of our clients are overwhelmed with caring for their loved one; they need assistance navigating the health care system and they are so appreciative of our ability to manage and keep family at home. As a member of National Association of Social Workers, my goal is to bring organization, compassion, crisis intervention, support, and family-caregiver training/guidance to my clients' lives.

In the 26 years in the field, my experience comprises work in many different behavioral health settings, including individual and family therapy, home based mental health treatment, and assessment for inpatient mental health and addiction treatment. Our clients' families often tell us how much they rely on coordination and flexibility.
